NOTES

1. As there are six vacancies on the Committee, you are entitled to vote for a maximum of six candidates but you may vote for less.
2. This is a vote so even if you intend attending the Annual General Meeting in person you should complete this Ballot Form.
3. For your vote to be valid you must vote for not more than six candidates and you must complete the Ballot Form with your membership number and name.
4. This Ballot Form is to be submitted only once.

The last date for receipt is no later than 9am Tuesday the 17th May 2022.
